Please send your purchase order (if applicable) with a completed Cognistat Assessment System Mail-in Order Form. 

Orders fulfilled only with a completed order form and pre-payment.  A Purchase Order without a pre-payment or completed Cognistat order form will not be processed.

VA Hospitals
